Disabler's Desk - I am a Retd.  Sr. Citizen (72+yrs), CVA, Hemaparasis Disabled (60%), right side of body fully paralyzed, cannot move outside without an escort but can only operate computer / internet with unusual left hand to communicate rest part of world especially in connection with social welfare works for betterment to old age disability and disabled persons Received 17 Railway Awards during later part services at Metro Railway-Kolkata - Chinmay Kar 

The captioned Chinmay Kar, extreme Disabled Person, who can’t move outside without escorts desire to visit Puri, Orissa on religious ground, like visiting Shri Shri Jagnnath Temple during next Oct-Nov’2017 period along with wife & attendant cum escort essential to disabled person certified by Govt Medical Doctors.For this reason I require 2 AC 2Tier Berths [For self & my wife aged 66 yrs] and one attendant cum escort in Sleeper class against my Post retirement 1^st class complementary pass issued by GM / M .Rly, Kolkata. I am agreeable to pay the cost of meal charges payable for availing Duronto Express.

Journey by Sealdah Puri Duronta Express up to Puri preferred only because this is only [ONLY] train available from Sealdah Station to Puri and the most uncomfortable road journey from Sealdah to Howrah, particularly up to Howrah Rail Station platform by an aged extreme Disabled Personcan not move out without escorts, can be avoided . I am however agreeable to pay the extra cost for meal charges payable. Incidentally the system proposed is in existence for Rajdhani Express HWH – Kolkata.

General Manager, South Easter Railway is earnestly requested to consider the request of an extreme Disabled Person and approve proposal as a special case and oblige.

The undersigned, an aged retired extreme Disabled, can’t move without escorts (as detailed at the top of letter head) had to submit before you for kind consideration and pass suitable orders:-

That the undersigned extreme Disabled is desirous to go to Puri exclusively for pilgrimage purpose by Sealdah Puri Duranto Express (to avoid trouble sum journey up to Howrah Station platform by an extreme Disabled Person like me) against his 1^st Class Rly Pass along with wife aged 66 yrs, an Asthma Patient and one attendant well conversant with requirements of a Hemiparasis Disabled during next Oct-Nov’17 period.

Under the circumstance it is requested to kindly examine the proposal & if there is any bar to get reservation by Duranto Sealdah Express and if so kindly suggest the best possible way to avail the train by this extreme Disabled Person as a special case and oblige.  All the testimonials required in support of factual position may kindly be seen at attachment.
